Hiring Forms

Please complete the following forms and email them as attachments in one email before your first day to humanresources@fvrd.ca. Alternatively, print and complete the forms and bring them on your first day.

The TD1 Provincial and TD1 Federal must contain your written signature and therefore paper copies will be required for these forms only. Digital signatures are not accepted.

What To Bring On Your First Day

Please bring all of your completed paperwork (Hiring Forms), plus your:

  • Driver’s license or BC ID
  • Social insurance card or letter 
  • Void cheque or an online form from your bank to set up direct deposit.

As per our Driver Licence Abstracts Policy, please forward your Driver Licence Abstract (Personal Driving Record) to Human Resources.  The simplest way is to go online at https://onlinebusiness.icbc.com/clio/. You will complete the necessary information and request the “Personal Driving Record” which is the Driver’s abstract.  Please send your abstracts to humanresources@fvrd.ca.

What to Wear to Work

Depending on your role with the FVRD, what you wear to work will vary. Below are some guidelines to help ensure that you always come to work prepared for the day and looking professional:

  • As representatives of the FVRD, dress and appearance should present a professional or identifiable appearance for customers, suppliers and the public.
  • The accepted standard for office staff should be business casual to professional attire.
  • Business casual or professional attire may be expected anytime an employee attends scheduled meetings, seminars, or works with the general public, outside business representatives and elected representatives.
  • Exceptions are allowed for Friday Jean's Day and FVRD events such as Halloween, etc.
  • Clothing for maintenance or outside work should promote safety and comfort and maintain a clean and professional appearance.
